Open the Templatize tab. Create a template with a name, issue type, summary, description, labels and optional sub-tasks. Make it available to one project or all.
Click Use template to create a pre-filled issue (and its sub-tasks) in the current project. The app handles required fields safely and tells you if a field needs to be added to the create screen.
